If you are visiting our office for the first time, please plan to arrive 15 minutes prior to your scheduled appointment time to allow time for registration. Please bring the following items with you: - Insurance card
- All current prescription medications (or list)
- Completed Registration Form (see link below)
- Completed Medical History Form (see link below)
- Signed Notice of Privacy Practices (see link below)
If you need to cancel or re-schedule an appointment for any reason, please notify us at least 24 hours in advance. If you fail to provide adequate notice of change in appointment, you may be assessed a $25 cancellation charge. Prescription Refills: If you have run out of refills on your prescription(s), refill requests should be made through your pharmacy. Certain medications (e.g. Accutane) require periodic visits for refills. In general, if it has been 6 months since you last visited our office and would like a prescription refill, we require you to schedule a follow-up appointment. At Insight Dermatology, we recognize the importance of your time and try our best to stay on schedule. If, however, we are running behind due to special circumstances, we apologize for any inconvenience we may have caused. Links to forms:
